PRIVACY / HIPPA STATEMEMT
Our agency is dedicated to maintaining the privacy of our Consumers
individually identifiable health information as protected by law, including the
Health Information Portability and Accountability Act (HIPAA). In conducting
our business, we will create records regarding our Consumers and the treatment
and services we provide to them. We are required by law to maintain the
confidentiality of health information that identifies our Consumers. We also
are required by law to provide our Consumers with a notice of our legal duties
and the privacy practices that we maintain in our agency concerning your
Protected Health Information (PHI). By federal and state law, we must follow
the terms of the notice of privacy practices that we have in effect at the

Consumer Rights
Under HIPAA, an individual has the following rights with regard to
his/her PHI:

The right to authorize the use and disclosure of PHI for certain non-TPO
purposes and for psychotherapy notes.

The right to receive a copy of the organization's Notice of Privacy Practices.

The right to request restrictions on certain uses and disclosures of his/her
PHI.

The right to request restrictions on how the organization communicates PHI to
the consumer.

The right to inspect and copy PHI.

The right to request an amendment of PHI.

The right to an accounting of the disclosures of PHI made by the covered entity
for purposes other than TPO and not pursuant to a valid authorization.

The right to complain about alleged violations to Friendship's Privacy Officer
and Department of Health and Human Services.
